- Marital Status
- Father and Mother's Full Names
- Years of Education
- Residential Information
- Employment History
Items you will need to gather (deceased):
- Clothing (Shoes, undergarments, etc.)
- Eye Glasses, jewelry, dentures, rosary
- Recent photograph
- Military discharge papers (if applies-form D.D. 214)
- Cemetery information
